New Patient Form
Please complete and submit this form prior to your consultation at Synergy Integrative Specialty Veterinary Clinic (SISVC).
New Client Policy:
Due to the increased demand for appointments, we are now requiring a non-refundable deposit of $75 for all New Client consultations at the time of scheduling. We reserve 2 hours for new patient consults, and no shows and last minute cancellations are becoming an issue and taking spots from others who need to have their pets seen.
If you call to reschedule 24 business hours or more in advance, the money will be used towards the new appointment. If you fail to keep your appointment or contact us 24 business hours or more before your appointment time, this money will be forfeited to SISVC. This policy has become necessary to allow us to see new clients in a timelier manner and accommodate the increased demand for services as we grow.
If your pet is currently under the care of your primary care veterinarian or a surgeon, you may ask them instead to complete and submit the Veterinarian Referral Form.
Please have all lab work and radiographs electronically sent to SISVC 72 hours prior to your scheduled appointment.
